*All prizes must be accepted as awarded. All prizes are non-transferable. Inability to accept or deliver prizes will result in forfeiture.
*Before any prize is awarded, contest winner must provide Chesapeake Eye Care & Laser Center with valid headshot photo to confirm photo of eyes matche headshot. Failure to provide this identification may result in forfeiture of prizes.
*By participating, all participants and winner grant Chesapeake Eye Care & Laser Center and 102.7 Jack FM and Mix 106.5 permission to use their names and photographs in connection with the promotion (via print, broadcast, internet or other forms of media) of this contest and waive any claims to royalty, right, compensation or remuneration for such use.
*Winner, to the extent applicable, agrees to be bound by any and all blackout periods, space availability or other conditions or limitations stated by Chesapeake Eye Care & Laser Center, 102.7 Jack FM and Mix 106.5 or its contest prize partner, in association with the prize relating to a hotel and airline.
